Collection of Community Nursing Indicators (modified from previous collection as Welsh Core Indicators) |
From March 2004 - 8 Community Nursing Indicators, previously collected under WCI (which has been discontinued), will be collected as Community Nursing Indicators.
Five indicators are being collected via a replacement data collection; the remaining 3 are continuing to be collected via other existing data collections.
The 5 Community Nursing Indicators being collected via the new Community Nursing Indicators data collection are: -

Indicator 3 |
Number of children 'in need' (as defined in 1989 Children's Act – see below) aged under 5 years as % of children under 5 years on Health Visitor caseloads at 31 January |
Definition of Terms |
Definition of 'children in need', 1989 Children's Act: "he is unlikely to achieve or maintain, or to have the opportunity of achieving or maintaining, a reasonable standard of health or development without the provision for him of services by a local authority; his health or development is likely to be significantly impaired, or further impaired, with the provision for him of such a service, or he is disabled." (section 17(10)). Children under 5 years on Health Visitor caseloads: all children under 5 years. Position at 31 January following annual profiling (caseload count). |

Indicator of level of need in a defined area. Workload of Health Visitors may be related to social deprivation indices – a social exclusion indicator. |

Definition of Terms |
Include venous ulcers only. Include each separate ulcer.
RGNs in District Nurse team: include all RGNs and DNs. |

Application of research based diagnosis and treatment methods demonstrate improved healing rates. Value for Money indicator. |

Definition of Terms |
DN service: include all staff in DN service – both qualified and unqualified. District Nurse caseload: number of patients seen by the District Nursing service (both qualified and unqualified nurses). Exclude patients requiring more than 1 visit per day solely for the purposes of administering eye drops.

Definition of Terms |
District Nurse caseload: number of patients seen by the District Nursing service (both qualified and unqualified nurses). Pressure sore grading system is system used locally.

Definition of Terms |
Care package – resources identified to maintain patient at home. May include home adaptations, equipment, drugs, health or SSD home care staff time, carer time. Inadequate – where resources, identified above, are needed to meet the full care needs of a patient discharged home, have either; 1. Not been identified at all prior to discharge, or 2. Have been underestimated, or, 3. Have been identified but are not available on discharge. |

Inadequate discharge arrangements may lead to inappropriate re-admission to acute care and delayed discharge. Indicator of poor interagency or inter/intra Trust partnerships. |

Percentage of babies breastfed at age 2 months or at first immunisation |
Definition of Terms |
Number of babies breastfed (wholly or partially) at age 2 months post-natal or at first immunisation of baby as a % of all babies age 2 months at some time during the year 1 April to 31 March. Includes partial breast-feeders i.e. any breast-feeding or the giving of breast milk to infants. |
