Information Requirements

The information requirements to be reported to Welsh Government are described in the associated reporting specification:

Performance Indicators for Hearing Screening Pathway (by Health Board)			Reporting period: For a given academic cohort, screening that is completed in the period 1^st September of ‘Reception’ to 30^th November of ‘Year 1’. Report due by 31^st January of ‘Year 1’.

9	Total number of eligible and consenting children ordinarily resident in the health board area who have completed the screening pathway		Health Boards are obliged to collect this data: number who have gone through the screening pathway. 9 will be all who have completed the pathway.

In addition to the indicators, please add narrative relating to any issues you have encountered in ensuring that children ordinarily resident in the health board area but on school roll outside the area, or 'educated other' and not on school roll are captured within health board data. Please also comment on other data fields that you consider may be useful in future, or any problems you have encountered with data collection.
