The proforma 'CDSWR' should be completed and returned via the Welsh Government (WG) Knowledge & Analytical Services secure online transfer mechanism Access Files Online (AFON). For details of how to access and use AFON please use the contact details below.
The form should be returned annually by the end of July / beginning of August each year, with the return relating to the period of the previous financial year. Exact dates of submission will be advised by Knowledge & Analytical Services, Welsh Government.
