GP Practice Cluster Code

This is the code for a GP Practice Cluster.

Format: 5-character alphanumeric

Structure

•     Each GP Practice Cluster Code begins with the letters ‘CC’.

•     The 3^rd character provides a relationship to the parent Local Health Board (LHB)

o  All GP Practice Cluster Codes starting with CC1 will relate to Betsi Cadwaladr University LHB (7A1);

o  All GP Practice Cluster Codes starting with CC2 will relate to Hywel Dda LHB (7A2);

o  All GP Practice Cluster Codes starting with CC3 will relate to Abertawe Bro Morgannwg University LHB (7A3);

o  etc.

•     The 4^th and 5^th characters are a sequential number identifying the individual GP Practice Cluster.
